Reward & Conduct - allow Field Types in the multiple entry grid tool Merged

Simply add the 'field type' drop down (if one has been set up) to the grid tool format to allow them to used if rewards & sanctions are being entered on mass. At the moment, 'field types' can only be entered if you enter items one at a time.
